Shrewsbury and Telford Hospital NHS Trust has been fined more than £1.3m (€1.5m; $1.6m) at Telford Magistrates’ Court after admitting failures in care that contributed to the deaths of a pensioner with chronic lung disease and a patient in his 30s receiving dialysis.
Max Dingle, 83, died after becoming trapped in a bariatric bed. He had a cardiac arrest after he was freed. Mohammed Ismael Zaman, 31, known as Bolly, died of severe blood loss while undergoing dialysis after his line became disconnected. The trust was prosecuted by the Care Quality Commission (CQC) under the Health and Social Care Act 2008.
